ED Care ManagerTo use critical thinking skills to influence the efficient direction of patient care for patients in the Emergency Department.

To coordinate and implement strategies made through a collaborative process between the Enterprise Care Management team in a timely manner.

To ensure that quality health care services are being delivered to meet an individual’s specific healthcare needs in a cost effective manner.

To facilitate the appropriate utilization of resources and transition of patients to the most appropriate level of care along the continuum, through efficient communication between internal and external parties.Position Requirements:Bachelors Degree (BA, BSA, BSN, etc.) or equiv required and34 years’ recent experience in care managementBoard Certification by 12/31/2015 or within two years of hire Global perspective of changing healthcare environmentAbility to prioritize workBasic computer skillsEffective interpersonal communication skillsUtilization of critical thinking in timely decisionmaking Current Illinois RN licenseCPR Certification Flexible hours according to customer needs.Ability to take weekend/holiday call.Ability to demonstrate a commitment to patientfocused care.Ability to prioritize independently and respond to multiple simultaneous requests.Ability to work in a fastpased environment with established time constraints and in difficult situations.May be exposed to hazardous materials and lifethreatening diseases.This position needs to recognize needs and behaviors of specific age groups of patients treated.Ability to transfer weight of laptop (up to 20 lbs.)If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015.

Ability to lift up to 35 pounds without assistance.

For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.

Unique patient lifting/movement situations will be assessed on a casebycase basis.Accountabilities:Assist in the development, implementation and evaluation of a multidisciplinary individualized patient plan of care that includes an appropriate transition out of the ED or occupancy of a hospital bed.Ensure Clinical Documentation and Cost Effective Use of Hospital ResourcesParticipates in ongoing performance improvement initiatives to enhance outcome for defined patient population.Responsible for Profession growth.
